Philadelphia Mental Retardation Services is responsible for planning, administering, monitoring and coordinating services. It was created under the 1966 State Mental Health and Mental Retardation Act. The annual plan provides detailed information about services and supports and future plans with fiscal data.

Services Offered: BARC was founded in 1951 providing life-long services. The early intervention service supports families and assists birth to five year olds in Bucks, Berks, Schuylkill, Counties, and Delaware at no cost to families. The residential programs are for those individuals who desire a more independent life in homes and apartments in Bucks County (twenty-four hour supervision, semi independent living, family living, independent living with thirty hours supervision/week) fully subsidized or private pay ranging from $30- $150k per annum. The vocational services department strives to attain a useful and productive life for everyone through two sheltered workshops in Bucks County and a community employment division that is fully subsidized or privately paid ranging from $25- $50 per diem. The department also provides a Senior Citizens Retirement program for those individuals who no longer wish to work.

Services Offered:
Since 1992, the ChildLink Program has served as the single point of entry in Philadelphia for all children from birth to three and their families in need of early intervention services because of a developmental delay or disability. ChildLink also provides early intervention service coordination to eligible children and serves as a point of contact for children three to school age in need of early intervention services. Families and professionals can easily access early intervention services directly by calling 215-731-2110.

ChildLink serves all areas of the City and at no cost to parents. Services for eligible children are developed and authorized during an Individualized Family Service Plan (IFSP) process by the service coordinator and primarily provided in the family’s home and community. Services are provided by a wide range of organizations in areas such as special instruction, physical therapy, occupational therapy and speech therapy. Specialty services are also provided for children who are blind, deaf or have other special needs.

Under contract with the City of Philadelphia’s Office of Mental Retardation Services, ChildLink is administered by Philadelphia Health Management Corporation, whose mission as a public health organization is to improve the health of the community through outreach, education, research, planning, technical assistance and direct services. Services Offered: Founded in 1991, Community Integrated Services (CIS) is a premier provider of community-based work and alternative day services for individuals with developmental disabilities in southeastern Pennsylvania and Delaware. The success of the people it serves at finding and keeping jobs and their overall involvement in their communities is rooted in the agency’s values:

CIS’ main office is on Delaware Avenue in the Northern Liberties section in Philadelphia. CIS’ services are community based, so its 17 staff members are more commonly found working with our consumers in the communities where they live and work.

Our job is to work with each person to figure out how to overcome obstacles so they can achieve the goals they have set for themselves. Our focus is to support people as they find and keep jobs in the community, and become involved in meaningful community activities during the day.

CHILDREN’S MENTAL RETARDATION SERVICES

CHILDREN’S EARLY INTERVENTION PROGRAM, 26 S. 40th Street. The Children’s Early Intervention Program works with family and community to provide services for children with developmental delays in pre-school, community and home settings, improving quality of life by building skills in the social, motor, cognitive, communication, and adaptive areas.

MENTAL RETARDATION UNIT, 26 S. 40th Street. The Mental Retardation Case Management Unit locates, monitors, and coordinates programs and services for the support and training of persons with mental retardation.

CHILDREN’S RESOURCE COORDINATION, 137 South 58th Street. The Resource Coordination Unit provides assistance for children up to 17 years of age with mental illness currently placed in treatment facilities.  All referrals are made and approved through the Office of Mental Health/Mental Retardation.

CHILDREN’S TARGETED CASE MANAGEMENT, 137 South 58th Street. This program provides individualized in-community support for children (up to 17 years of age) with serious mental illness. All referrals are made and approved through the Office of Mental Health/Mental Retardation.

Barber Resources is a division of The Dr. Gertrude A. Barber Center, Inc., a charitable, non-profit organization which was founded in Erie, Pennsylvania in 1952 and now serves over 2,000 individuals. Services Offered: EMAN Community Living Inc. is a community based residential living program that currently is serving a population of 53 adult men and women with mental retardation as a primary diagnosis. We have 13 CLA sites, 6 SLA sites, and we support 11 Life Sharing thru Family Living Placements. The CLA’s are located in Mt.Airy/Germantown in Chestnut Hill and the Roxboro areas of the city. The SLA sites are located in Mt.Airy.

The Family Living sites are spread throughout the city. The Cost of service in determined by the ISP, which identifies the individual’s personal choice, wants and needs. Our agency is dedicated to serving individuals in primary need of residential service. Many of our Board members are family members. We have a strong sense of community interaction and integration. Our Community Supports Program supports our community interaction. We have a very detailed pre-service training program and we are always meeting the new challenges presented by the physical and mental health needs of the people we support.

Services Offered: Family Support Services provides Early Intervention services to children Birth to Five years old. We have two locations for services to families. Our community site is located on Chestnut St., which serves Birth to Three children in their natural environments and Three to Five children in their community programs or homes in the Southwest/West area of Philadelphia. Our other location is a center-based program for three to five children, located near the Art Museum on Hamilton St. All programs evaluate children and either write or participate in the writing of the service plans. Founded in 1952, Horizon House's supports for persons with mental retardation are designed to increase their ability to manage their lives, and to facilitate active involvement and integration into the community.

Residential
Options include community homes, small group residences with round the clock, in-home assistance; semi-independent or supported living arrangements for individuals who need less than 24-hour per day in-home assistance and who live in their own homes or apartments; and life-sharing, for those desiring a more family-oriented living situation.

Life-Long Supports
Horizon House also offers community integrated employment, financial planning and management services, and retirement planning. The Active Retirement Program provides specialized supports to people with mental retardation living in nursing homes. Services Offered: Located in all five counties of Greater Philadelphia, JFCS provides a comprehensive range of counseling and support programs and serves more than 35,000 children and adults each year.

JFCS services for children and adults with special needs and their families include counseling, case management, planning for the future, life-skills transition, support groups, socialization programs, advocacy and information/referral. Services are available throughout Greater Philadelphia. Fees are based on a sliding scale.

JFCS has a contract with the City of Philadelphia Office of Mental Retardation to provide respite services to developmentally delayed children and adults. The respite homes are certified by JFCS, and matches are made by the JFCS Coordinator of Respite Services. The City of Philadelphia Base Service Units authorize and make the referrals for this program. The program serves children and adults throughout Philadelphia. The child/adult must be registered with his/her Base Service Unit. There is no cost for parents, but they must provide transportation to the respite home.

Programs Employing People - PEP - offers a range of , vocational, educational, job placement social, and recreational services to individuals with developmental (and some) physical disabilities. Founded in 1969, the agency serves Philadelphia adults of all ages and both sexes (along with Camp PEP for children in summer). Primary services are vocational (community job placement, job coaching, pre-employment education & literacy, on-site production-contract workshops); and adult-day and social/recreational activities, both in-house and throughout the community. No individual with a developmental disability is turned away on the basis of inability to pay; service fees are handled case-by-case, often through coordination with other agencies.

Resources for Human Development, Inc. (RHD) is a diverse and creative non-profit cooperation that has been providing social services since 1970. The mission behind our work is to support the human creativity and concern for others that in turn leads to the best service possible. Through it's commitment to the values of personal dignity, collaboration with consumers, families and significant others, comprehensive array of services, and flexibility of program design, RHD provides a highly effective avenue to successful community integration for people with developmental disabilities.

Currently, RHD residential programs are serving over 300 people with developmental disabilities. This number includes over 90 individuals in group homes, 3 individuals in supervised apartments, and 23 individuals in supported apartments. RHD provides a variety of day services for more than 175 consumers. The characteristics of our residential consumers are as follows: severe behavior issues, seizure disorders, Physicals disabilities, e.g. cerebral palsy, incontinence, paraplegia. aphasia, health issues, mental illness, and substance abuse. These individuals, who range from mild to severe mental retardation, live and work in a variety of settings.

RHD has established residential and day programs for persons with mental retardation. The key to these services is flexibility, based on consumer needs: RHD services range in intensity from 10 hours per week to complete wrap-around residential/day services that are provided on a 24hour/7 days per week basis. Services are created in response to individual need and assessment.

Services Offered: Provides monitoring services and advocacy support for people who receive Mental Retardation Services in Philadelphia. We visit people in their residential, vocational, or community settings to conduct a survey that assesses the quality of services they receive and their satisfaction with that support. These visits also include people who are under court protection from the Embreeville Class, which the city awarded to us for review and safeguard. We conduct the Pennsylvania Waiting List Campaign project, and also have acquired a grant to provide HIV/AIDS training for people who have mental retardation and are interested in learning about AIDS prevention.

Services Offered: We provide residential services for individuals with developmental disabilities age 16 and older and offer a secure, family like setting where a daily routine is established upon arrival. Individuals are given the opportunity to participate in all activities to their abilities. The Walker Center is based in West Philadelphia but has homes within the city border as well as Delaware county and Montgomery county. Our specialty has been residential programming and respite care. We have been in existence since 1945 but remained small to maintain quality cost and care.

The Walker Center is a home that provides residential and respite care services to individuals with developmental disabilities. The Walker Center welcomes long-term consumers to a safe, nurturing home environment while encouraging each person to become as independent as possible. The Center promotes the basic human rights, dignity, health, and safety of the people we serve.

RESIDENTIAL PROGRAM

The Walker Center involves the persons served in our residential homes in individual planning, decision making, and implementation of the services they will receive. This evidenced by the Residential Policies and Procedures Handbook regarding the Individual Care Plan, by the annual Consumer Satisfaction Survey, monthly individual program consumer meetings, the use of the Consumer Advisory Committee, and by the Board participation. The Walker Center uses a team approach, provides coordinated, individualized, outcome oriented services, leading to desired outcomes.

RESPITE CARE PROGRAM

The Walker Center's Respite Care Program provides family members and caregivers relief during emergencies, illnesses, holidays and vacations. Reservations for the Respite Care Program are required and the fee is based upon a per diem rate for services available upon request. There is an addition fee for transportation and activities. Spending money may be required to participate in some outings. The Walker Center provides direct assistance in personal hygiene, community inclusion, leisure activities and self-directed programs.

The daily rate is a sliding scale based on the need and the individuals we support. The scale begins at $159.78 per day.

OLDER ADULT DAY PROGRAM

The Walker Center's Adult Day Program is open to adults 60 years of age and older with disabilities and /or Alzheimer's. This program offers activities that address participants' interrelated needs, interest and abilities. Each activity is developed around the group's health condition and age with the outcome of enhancing each individual's quality of life.

The daily rate is a sliding scale beginning at $55. The Center can provide assistance with transportation for a fee. Funding may be through private pay, funding agencies (waiver) or fee for service.

Services Offered: WES Health Centers (WES), founded in 1992, is located in North Philadelphia. WES provides Mental Retardation, Mental Health, and Substance Abuse Services to children, adolescents, adults, and families. Mental Retardation Services include: Support Coordination (Case Management), Early Intervention, both center-based and home/community, and Family Driven Support Services, In-home supports, and Adult Day Services.

The Early Intervention Program provides cognitive learning and/ or therapeutic (speech, occupational, physical) interventions for children birth to five (5) years of age.

Family Driven Support Services are designed to provide families with a vast array of resources and minimal dollars that continue to enable families to maintain their relative with mental retardation at home.

WES has two components within Adult Day Services (Individuals 18 and over): Vocational Habilitation Program (VHP) and Community Integrated Employment (WESWorks). The VHP is a Sheltered Workshop designed to teach appropriate work skills, appropriate socialization, regularity of attendance, importance of work completion, and job readiness skills. WESWorks is a supported employment program designed to provide on the job training using a one on one job coach to provide intensive training until the job skills are mastered.
